Our objective was to identify the complications of diabetes, comorbidities, and the factors affecting the patients living in rural areas of Miandoab, Iran. In this cross-sectional study, 459 patients were recruited randomly from thirty-eight health centers of 108 existing health centers. Diabetic patients from each health center by a simple random method were selected. Data were collected using a three-part descriptive questionnaire. Variables of age, sex, type of treatment, education, blood pressure, cholesterol, duration of disease, fasting blood glucose, and cholesterol are fundamental causes of diabetes complications ( p < 0.05). Hypertension is the most common disease associated with diabetes, and the number of complications of diabetes is the most significant cause of the hospitalization of diabetic patients ( p < 0.05). It was necessary to strengthen inter-departmental cooperation, strengthen information infrastructure, optimize existing structures and capacities, especially human resources and health network structure, and focus on the quality implementation of the family physician program.
