531 Prevalence of condyloma acuminata in sexual and gender diverse populations: A retrospective cohort study

Data on the prevalence of condyloma acuminata (CA) and Human Papillomavirus (HPV) vaccination among sexual and gender diverse (SGD) patients are limited. We sought to determine HPV vaccination rates and CA prevalence among SGD patients compared to cisgender, heterosexual counterparts through a retrospective cohort study using electronic health records from 46,507 adult patients seen at a community health center between August 1, 2014 and August 1, 2020. Adjusted risk ratios (aRR) and 95% confidence intervals (CI) were calculated using log-binomial regression adjusting for age and HIV status. 24.83% of patients at our institution received at least one HPV vaccination dose. When stratified by cohort, 28.26% of 10,499 cisgender, heterosexual women, 13.99% of 8,337 cisgender, heterosexual men, 27.69% of 13,836 MSM, 22.52% of 2,165 WSW, and 30.29% of 3,656 TGD patients received at least one HPV vaccine dose. MSM had an aRR of CA of 3.56 (95% CI: 3.06 – 4.14) compared to cisgender, heterosexual men. In contrast, there was no evidence of difference in CA prevalence between WSW and cisgender heterosexual women (aRR: 0.87; 95% CI: 0.57 – 1.33). Compared to cisgender, heterosexual men and cisgender, heterosexual women, respectively, both transmasculine and transfeminine patients had lower aRRs of CA at 0.15 (95% CI: 0.08 – 0.30) and 0.51 (95% CI: 0.37 – 0.71), respectively. Our analysis suggests that CA risk may be modulated by behavioral factors related to sexual and gender identity, thus highlighting a need for additional preventive care in special populations such as MSM.
