Concordance in multimodal prognostication after cardiac arrest: Improving accuracy or comparing apples to oranges?

“In dubio abstine”. This famous latin saying certainly applies to the prediction of neurological outcome after cardiac arrest. The majority of comatose patients that are admitted to the intensive care unit (ICU) after resuscitation from cardiac arrest, die from withdrawal of life support therapy (WLST) because of an anticipated poor outcome. Accuracy of prognostication is essential, both to prevent inappropriate WLST and prolonged treatment of patients without prospects of a good recovery. The ERC-ESICM guidelines for prognostication advice a multimodal approach including clinical examination, electrophysiology, biomarkers and imaging.
