P2-304Survey of end-of-life medical care of lung cancer

Background: Lung cancer is the leading cause of cancer related death. Although the majority of cancer patients want to die at home, it is inferred that there are still many cases dying at hospital due to various factors such as delays in improving of at-home medical care environment and the psychological and physical burden of caregivers. In this research, we aim to investigate the actual condition of end-of-life medical care of lung cancer and clarify the improvement points. Methods: In the period from January 2009 to June 2014, among the 315 cases diagnosed as advanced lung cancer in our department, 195 patients whose death was confirmed were examined. Backward analysis was performed based on the information of medical records. Results: Median age: 67 years (38 to 97 years), Clinical stage: III/IV = 49 (25.1%), Sex: male / female = 151 (77.4%)/44 (22.6%), Tissue type: NSCLC/SCLC and LCNEC = 148 (75.9%)/47 (24.1%)/146 (74.9%), PS at diagnosis: 0-1/2/3-4 = 152/31/12, Number of treatment regimen: 1/2/3/4 or more = 57/44/42/52, Median survival time: 360 days (15 to 2477 days). Comparing the group who died at our hospital and the group who died at home, a trend that the overall survival time was significantly longer (p = 0.028) in the at-home death group. Conclusions: Especially for diseases that are unlikely to be expected of long-term prognosis like lung cancer, efforts to confirm the patient's home death hope from the early stage are necessary.
