•The Endotoxin Activity (EA) assay allows the rapid determination of whole blood endotoxemia;•Endotoxemia is common in the early phase of septic shock of any origin;•Patients with positive cultures for Gram-Negative bacteria showed a higher mean EA.•An elevated EA within 24h of shock onset was associated with an increased severity of illness.
