Adverse events of biologic therapies]

Biological therapies have revolutionized the management and the outcome of patients with inflammatory rheumatic diseases. The prescription and the number of patients under biotherapies have greatly increased. Unfortunately, these treatments are occasionally accompanied by side effects usually reversible upon treatment discontinuation. Some are preventable or can be reduced, including through the screening and prevention (tuberculosis screening, vaccination…). It is important to be aware of these different side effects to adapt the prescription of biologics according to the patient's profile (history of infection, malignancy, heart failure…). It is also critical to involve the patients into the decision-making and thus, to inform them thoroughly.Les biologiques ont révolutionné la prise en charge et l’évolution des patients atteints de maladies rhumatismales inflammatoires. Leur prescription et le nombre de patients sous biothérapies ont largement augmenté. Malheureusement, ces traitements sont occasionnellement accompagnés d’effets secondaires, le plus souvent réversibles à l’arrêt de ceux-ci, qui inquiètent le prescripteur et le patient. Certains effets sont cependant évitables ou peuvent être réduits, notamment grâce au dépistage et à la prévention (dépistage de la tuberculose, vaccination,…). Il est important de connaître ces différents effets secondaires afin d’adapter la prescription des biothérapies au profil des patients (antécédents d’infection, de néoplasie, d’insuffisance cardiaque,…). Il est également très important d’impliquer le patient dans la prise de décision et de l’informer de façon aussi exhaustive que possible.
