Cross Cultural Adaptation And Validation Of Lupuspro In Southern Chinese Patients With Systemic Lupus Erythematosus

Background LupusPRO is a disease targeted patient reported health outcome measure that was developed and validated for use among US patients with Systemic Lupus Erythematosus (SLE). It has been cross culturally adapted and validated in other languages. Objectives To present results of cross-cultural adaptation of LupusPRO into Chinese language, and test its psychometric properties among southern Chinese patients with SLE in Hong Kong Methods LupusPRO was translated into “Traditional” Chinese using forwards and backwards translation methods, followed by pretesting among five native Chinese speakers. Based on the feedback obtained, the translated tool was revised until no issues on feedback from pretesting were noted. The Chinese language LupusPRO tool was administered along with generic Quality of life tool (MOS-SF36) to SLE patients meeting 4 ACR classification criteria. At the same time, demographic, clinical data (ACR criteria, medications), disease activity (SELENA-SLEDAI) and damage (SLICC-ACR/SDI) were obtained. We tested the reliability (Internal consistency reliability-ICR), validity (convergent (CV) and criterion validity), factor analysis of the Chinese LupusPRO. Weak correlation between LupusPRO and disease activity was expected due to a priori and unique PRO information captured by the tool. Results 463 SLE patients (95% Females) with a mean (SD) age of 42.3 (13.5) yrs participated. Ninety five percent were women. Mean (SD) disease duration was 9.9 (7.5) years. The mean (SD) PGA, SELENA-SLEDAI and SDI were 0.48 (0.45), 2.9 (3.0) and 0.7 (1.2) respectively. Sixty percent were on steroids and 64% on hydroxy-chloroquine. The ICR of the LupusPRO domains ranged from 0.60-0.94. LupusPRO domains had CV against corresponding SF36 domains (Table 1). Significant but weak correlation with disease activity was noted for criterion validity. Factor Analysis conformed to the model, except for the two Lupus Medication items and one item on “worry about income” showing cross loadings onto other domains.
