Transfusion Strategy in Myocardial Infarction and Anemia

To the Editor: In the Myocardial Ischemia and Transfusion (MINT) trial, Carson et al. (Nov. 11 issue)(1) addressed a crucial question in transfusion medicine regarding the effect of a restrictive transfusion strategy in patients with acute myocardial infarction. The medical community has been waiting impatiently for the results of this well-conducted trial. Nonetheless, the conflicting results between the absence of statistical significance (P=0.07) and the clinically meaningful difference (15%) in the risk of a primary-outcome event, along with similar trends in secondary outcomes in favor of the liberal strategy, raise questions about the conclusions of the trial.
