•Presentation of Rickettsias can be settling and atypical, but the diagnosis is very important.•Severe cases of rickettsia could lead to HLH.•HLH is an inappropriate response causing multiorgan damage.•A rare case of life-threatening infection by R. conorii with HLH was successfully treated with immunoglobulin, corticosteroid and doxycycline.
