Magnetic Resonance Imaging–Targeted Versus Systematic Prostate Biopsies: 2-year Follow-up of a Prospective Randomized Trial (PRECISE)

The PRECISE trial demonstrated noninferiority of magnetic resonance imaging (MRI) with targeted biopsy (TBx) if indicated to a systematic biopsy (SBx)-in-all strategy. This 2-yr follow-up for men who avoided a biopsy (MRI arm) or had a negative biopsy or grade group 1 on biopsy (both arms) revealed no difference in detection of clinically significant cancer between the arms, even though 38% of the MRI group avoided an initial biopsy and the median number of cores was lower for those undergoing TBx (3 vs 12 for SBx). These results support the safety and reliability of the MRI ± TBx approach.
