•Drug-induced immune haemolytic anaemia (DIIHA) is a rare adverse reaction.•Ceftriaxone is a commonly reported antimicrobial to cause DIIHA.•Clinicians must be aware of DIIHA by ceftriaxone even in ambulatory patients.•Weekly haematological monitoring is needed in ambulatory patients on ceftriaxone.
