•Transesophageal echocardiogram probe insertion in intubated critically ill patients can be difficult, leading to complications.•This randomized, clinical trial compared successful transesophageal echocardiogram probe insertion and the incidence of complications using either videolaryngoscope or conventional technique.•The use of a videolaryngoscope improved the first attempt insertion of the probe (90% vs. 58%) and decreased the number of complications (pharyngo-laryngeal mucosal injuries) (14% vs. 52%) compared with a conventional technique.•The most frequent site of pharyngeal injury in both groups was the posterior hypopharyngeal wall.
