•Despite the US Preventive Services Task Force recommending annual LDCT lung cancer screening for individuals aged 50-80 years with ≥20 pack-year smoking history, uptake remains low and may not be sufficiently prioritized by policymakers and clinicians.•Increasing LDCT cancer screening rates from current levels (12.5% overall) to full compliance (100% screening) with guidelines would reduce health disparities across education, income, and race/ethnicity groups and generate up to $182.1 billion in additional value in the US, where up to 32.2% of this value is due to reduction in health disparities.
