Thyroïdectomie par voie transorale (TOETVA). Recommandations de l’AFCE (Association francophone de chirurgie endocrinienne) avec la SFE (Société française d’endocrinologie) et la SFMN (Société française de médecine nucléaire)

La thyroïdectomie par voie trans orale peut être proposée chez des patients sélectionnés, porteurs d’une thyroïde < 45 ml et/ou d’un nodule < 4 cm en cas de lésion Bethesda II, III ou IV, ou < 2 cm en cas de lésion Bethesda V ou VI, sans suspicion d’atteinte ganglionnaire latérale ni extension médiastinale, souhaitant éviter une cicatrice cervicale, avec un état dentaire satisfaisant, et ayant reçu une information sur les risques spécifiques de la voie trans orale et la nécessité de soins buccaux périopératoires, comme sur l’absence de preuve de son efficacité en termes de qualité de vie et de satisfaction des patients. Il faut informer le patient de la possibilité de douleurs postopératoires cervicales et mentonnières pouvant persister de quelques jours à quelques semaines après l’intervention. La thyroïdectomie par voie trans orale doit être réalisée dans des centres experts en chirurgie thyroïdienne. Transoral endoscopic thyroidectomy vestibular approach (TOETVA) can be proposed for selected patients with a thyroid volume < 45 ml and/or a nodule < 4 cm (for Bethesda category II, III or IV lesions), or < 2 cm (for Bethesda category V or VI lesions), with no suspicion of lateral nodal involvement or mediastinal extension who wish to avoid a cervical scar. Such patients should have satisfactory dental status, have been educated on the specific risks of the transoral route and the need for peri-operative oral care, and also fully informed regarding the lack of proof of TOETVA effectiveness in terms of quality of life and patient satisfaction. The patient should be made aware of the possibility of post-operative pain in the neck cervical and chin, which may persist for several days to a few weeks after the intervention. Transoral endoscopic thyroidectomy should be performed in centers with expertise in thyroid surgery.
