The objective of this work is to analyze the profile of diseases related to inadequate environmental sanitation (DRSAI) in children under 4 years of age between 2015 and 2019. This is an ecological study with secondary data from the Information System database. Hospitals (SIH/SUS), the Brazilian Institute of Geography and Statistics (IBGE) and the Primary Care Information System (SIAB). The research was carried out in the Metropolitan Region of Greater São Luís (RMGSL), composed of 13 municipalities in Maranhão, in which a descriptive analysis was carried out of the hospitalizations of children aged 0 to 4 years in the period from 2015 to 2019, by municipality of residence, who had diseases related to inadequate environmental sanitation as the cause of informed hospitalization. A total of 3,339 hospitalizations for DRSAI were reported in the RMGSL. The municipalities of Santa Rita and Rosário were the ones that presented the most (82%) hospitalizations for DRSAI were hospitalizations for diarrhea. There is a predominance of hospitalizations of children between 1 and 4 years old, diagnosed with diarrhea and gastroenteritis of presumed infectious origin (1640), followed by other infectious diseases (703). As for the diagnoses that led to death, intestinal infectious diseases predominated (4). The study allows for an opening of questions, making one reflect on the importance of basic sanitation for children in early childhood.
