EP27.19: A pregnant woman with embedded IUD in uterosacral ligament

The increase in population without welfare facilities, especially in developing countries, has unfair consequences for individuals and society. One of the methods used to prevent unwanted pregnancy is the intrauterine device (IUD). Pregnancy with a displaced IUD is a great clinical challenge. Our case is a 26-year-old woman who underwent a Caesarean section eight months ago. During the third week after delivery, an IUD was inserted into her uterine cavity. A few months later, she was referred with menstrual retard and positive β-hCG. In the clinical exam, the IUD string was not found. In the transvaginal ultrasound (TVS), a pregnancy sac was found in the uterine cavity containing a live fetus with cardiac activity and the IUD was depicted outside the uterine cavity, in the left parametric surrounding by a thin rim of fluid in a direction suggestive of uterosacral ligament (USL). By laparoscopic examination, the IUD was removed, while was embedded in the left USL. TVS in postoperative state was performed to confirm the lack of complication (figure 1). To sum up, although the risk of pregnancy with an IUD placed in the uterine cavity is considerably low, we should keep in mind it would happen in the context of displaced IUD.
