EP07.24: Fetal brain anomalies in dichorionic diamniotic twin

In twin pregnancies, fetal brain anomaly is rare in both fetuses. Although acrania is not related to chromosomal abnormalities, holoprosencephaly is known to be associated with Trisomy 13 in 50% of fetuses. Termination of pregnancy is recommended if acrania or holoprosencephaly is detected before 24 weeks. In our case, we introduce a case in which brain anomalies were confirmed in both fetuses in dichorionic diamniotic (DCDA) twin pregnant with IVF. The patient was transferred to our centre because of both fetal brain anomalies on the 12 weeks 2 days of gestation. The heart rate of both fetuses was not confirmed on the ultrasound performed after visiting our hospital. Sonographic findings were lobar holoprosencephaly in one fetus and acrania in another fetus. After medical abortous delivery, both fetuses were fully expulsion. Gross features were same of sonographic findings. A chromosomal study was performed on both fetal tissues. Both fetuses in our case were confirmed as normal chromosome findings on the chromosomal analysis results, 46,XX, 46,Y, respectively. Because holoprosencephaly may be related to the maternal diabetes, alcohol consumption, and infection, the patient needs intensive care and high doses of folic acid from the time of preparation for the subsequent pregnancy.
