Background: Cutaneous adnexal tumors are rare neoplasms that arise from the skin's adnexal structures. Differential diagnosis includes benign and malignant entities. Case Report: A 62-year-old woman presented with a persistent nodular ulcerated lesion on the scalp. An incisional biopsy revealed a cutaneous adnexal tumor, which was excised. Histopathology showed epithelial neoplasia with positive immunohistochemistry for pan-cytokeratin AE1/AE3, gata-3, and estrogen receptors. It was suggested that it could be a DEC, and the patient remained disease-free after 14 months of follow-up. Conclusion: Here, we present a case of a 62-year-old woman with a nodular ulcerated lesion on the scalp, which was diagnosed as a cutaneous adnexal tumor, possibly a DEC, based on histopathology and immunohistochemistry findings.
