The Association of Uncontrolled Seizures with Health-Related Quality of Life by Age Group among Patients with Epilepsy (P4.101)

Objective: To examine the association between uncontrolled seizures and health-related quality of life (HRQoL) by age group among patients with epilepsy. Background: Though uncontrolled seizures affect HRQoL, little is known about how age may influence this relationship. Design/Methods: Data from the 2014–2015 US National Health and Wellness Survey, a nationally representative, self-administered, internet-based survey, were analyzed. Patients who reported more than 1 seizure in the past 6 months (N=562) were categorized into 18–34 (n=192), 35–49 (n=206), 50–64 (n=126), and 65+ (n=38) age groups. Demographics and health characteristics were collected. Outcomes included: SF-36v2 health domain scores and mental and physical component summary scores (MCS, PCS; higher scores=better health status); activity impairment via Work Productivity and Activity Impairment questionnaire (WPAI). Adjusting for patient characteristics, generalized linear models were used to compare differences in outcomes by age group among patients with uncontrolled seizures. Results: Among 562 respondents with uncontrolled seizures, 53.6% were female. The average number of seizures reported was 22.43 and did not differ significantly by age group. After adjusting for covariates (e.g., BMI, epilepsy severity), patients aged 65+ relative to those aged 18–34 scored lower on physical functioning (adjusted means: 38.41 vs. 46.79, p=0.017), role physical (35.97 vs. 43.96, p=0.020), and bodily pain (i.e., greater pain; 38.12 vs. 46.00, p=0.017) health domains. Patients aged 65+ compared with those aged 18–34 scored higher on MCS (47.22 vs. 39.38, p=0.04) and lower on PCS (35.66 vs. 47.05, p Conclusions: In this representative survey of patients with epilepsy, uncontrolled seizures negatively impacted mental health among younger patients, while, physical health progressively worsened with increasing age.
