The utility of screening for gestational diabetes mellitus (GDM) early in pregnancy to further reduce the risk of adverse pregnancy outcomes has been debated in recent years. Studies have shown that early screening and treatment of GDM are associated with reduced risk of preterm labor, cesarean section, and macrosomia compared with standard screening and treatment at 24-28 weeks. However, 2 recent randomized controlled trials found that early screening for GDM either did not lower or modestly lowered the incidence of maternal and neonatal outcomes. Given the current evidence, our aim is to compare pregnancy outcomes in GDM diagnosed early in pregnancy versus GDM diagnosed by standard oral glucose tolerance test (OGTT) in the second trimester.
