Direct specimen retrieval through the working channel of a therapeutic bronchoscope using a 1.1-mm cryoprobe

SESSION TITLE: Procedures Posters 4 SESSION TYPE: Original Investigation Posters PRESENTED ON: 10/10/2023 12:00 pm - 12:45 pm PURPOSE: While cryobiopsy requires serial bronchoscopic removal and insertion to obtain specimens, a 1.1 mm cryoprobe can be used to obtain small specimens through a guide sheath (GS) without bronchoscopic removal. We describe a novel technique to directly obtain cryobiopsy specimens through a working channel without bronchoscopic removal. METHODS: We conducted a retrospective observational study between September 2022 and March 2023 at a single institution. Patients with lung lesions who underwent forceps biopsy with 1.9-mm forceps (FB-231D; Olympus, Tokyo) followed by cryobiopsy with a 1.1-mm cryoprobe (No. 20402-402; Erbe, Tubingen) using a therapeutic bronchoscope (BF-1TQ290, 3.0-mm working channel; Olympus) were analyzed. For localization of the target lesions, a radial endobronchial ultrasound probe and a GS were used. After freezing (4-7 sec), the cryoprobe and the GS were retrieved en bloc directly through the working channel of the bronchoscope. RESULTS: A total of 11 patients with lung lesions were included in the study, each with a definitive pathologic diagnosis. Cryobiopsy solely provided a definitive pathological diagnosis in 2 cases (lung adenocarcinoma and methotrexate-associated lymphoproliferative disorder). The total number of specimens obtained by forceps biopsy and cryobiopsy were 75 and 33, respectively. The size of specimens obtained by cryobiopsy was significantly larger than that obtained by forceps biopsy (4.58 vs. 2.33 mm2, p < 0.001). No severe bleeding or pneumothorax occurred. CONCLUSIONS: Large specimens can be obtained safely by cryobiopsy using a 1.1-mm cryoprobe through the 3.0-mm working channel. CLINICAL IMPLICATIONS: Cryobiopsy can be performed effectively and safely without bronchoscopic removal using the present technique.
