User satisfaction and ease of use of single-use bronchoscopes for bronchoalveolar lavage

SESSION TITLE: Proceduralist Round Table SESSION TYPE: Rapid Fire Original Inv PRESENTED ON: 10/10/2023 12:00 pm - 12:45 pm PURPOSE: Bronchoalveolar Lavage (BAL) is a minimally invasive procedure that utilizes sampling of the lung by flushing sterile saline into an area of interest, and subsequently suctioning the solution for analysis. While this procedure has been a standard of care in pulmonology for nearly 50 years, evaluation of specimen collection and techniques, specifically within single-use flexible bronchoscopes (SUFB), has not been assessed. METHODS: To evaluate the ease of use and satisfaction of performing BAL, 7 interventional pulmonologists performed 2 simulated BALs on low fidelity lung models with 5 SUFBs (Ambu aScope 4 & 5 (with integrated sampler system), Boston Scientific Exalt B, Olympus H-SteriScope, and Verathon B-Flex (all of which utilized a lukens trap)). Once each physician completed BALs with all 5 scopes, a 9-question survey with a five-point rating system from unacceptable (1) to excellent (5) was given. RESULTS: Of the 9 areas evaluated, the aScope 5 rated highest in all categories and on average (4.9), followed by aScope 4 (4.5), Exalt B (3.7), H-SteriScope (3.3), then B-Flex (2.9). Furthermore, all physicians rated aScope 5 a 5.0, in overall sampling/assessment score (aScope 4 (4.4), Exalt B (3.7), H-SteriScope (3.0), and B-Flex (2.7)). Additional ratings include the following; overall comfort/ergonomics (aScope 5 (4.7), aScope 4 (4.1), Exalt B (4.0), H-SteriScope (3.4), and B-Flex (3.0)), overall ease of use and ease of setup ((aScope 5 (4.9) (4.7), aScope 4 (4.1) (4.4), Exalt B (3.9) (3.0), H-SteriScope (3.1) (3.0), and B-Flex (2.4) (2.6)), ease of sample collection (aScope 5 (5.0), aScope 4 (4.7), Exalt B (3.9), H-SteriScope (3.3), and B-Flex (3.0)), comfort of sampling without assistance (aScope 5 (5.0), aScope 4 (4.7), Exalt B (2.7), H-SteriScope (2.4), and B-Flex (2.1)), comfort of sampling with assistance (aScope 5 (5.0), aScope 4 (4.9), Exalt B (4.3), H-SteriScope (3.9), and B-Flex (3.6)), perceived safety of sampling (aScope 5 (4.9), aScope 4 (4.7), Exalt B (4.0), H-SteriScope (3.7), and B-Flex (3.4)), and satisfaction of sampling (aScope 5 (5.0), aScope 4 (4.3), Exalt B (4.1), H-SteriScope (3.4), and B-Flex (2.9)). CONCLUSIONS: Overall, aScope 5 rated highest amongst users across all categories related to ease of use and satisfaction as well as perceived safety for BAL. Both aScope 5 and aScope 4 which include the only integrated sampling system on the market, rated higher than all other single-use scopes when performing BAL. CLINICAL IMPLICATIONS: SUFBs with integrated sampling systems, such as the aScope 4 and aScope 5, provide clinicians with an easier and preferred tool for performing BAL. Integrated sampling systems may allow physicians to perform BAL more efficiently and safely, without the need for additional staff.
