•PS-OCT can detect abnormalities in the blood vessel wall associated with hypertension.•A new metric termed blood vessel birefringence index (BBI) is introduced.•BBI is able to distinguish between hypertensive and normotensive subjects.•PS-OCT-based BBI indicates high sensitivity and specificity when used as a biomarker.
