•Mutations in KRAS, STK11 and KEAP1 significantly co-occur in NSCLC.•KRASMUT + STK11MUT + KEAP1MUT significantly reduce OS independent of treatment regim.•KRAS G12C (regardless of co-mutations) is a negative predictive factor under ICI.•Triple mutations lead to more metastases, specifically bone metastases.
