•99mTc-Besilesomab is useful in the diagnosis of peripheral musculoskeletal system infections.•Semi-quantitative analysis with 99mTc-Besilesomab reduces inter-observer variability.•Semi-quantitative analysis with 99mTc-Besilesomab is reproducible.•The test is positive when the counts increase by 2 % in the delayed image.
