Molecular diagnosis of hand eczema

Because hand eczema is a diagnostic challenge even for experienced dermatologists, a correct diagnosis is essential to ensure success of specific therapies.Prerequisites for successful molecular diagnostics in general and in hand eczema in particular are discussed.Basic research and opinion statement on new developments in molecular diagnostics are considered with a special focus on hand eczema.The first molecular classifier to distinguish psoriasis from (hand) eczema signature has been introduced as CE-marked in vitro diagnostics (CE-IVD); many more biomarkers associated with diagnostics, theranostics, or natural course of the disease are currently being investigated.Diagnosis of hand eczema will be supported by molecular diagnostics in the near future; we are at the beginning of the molecular era in dermatology.HINTERGRUND: Das Handekzem ist auch für erfahrene Dermatologen eine differenzialdiagnostische Herausforderung; gleichzeitig ist die korrekte Diagnose Voraussetzung für den korrekten Einsatz spezifischer Therapien.Betrachtet werden die Voraussetzungen für den Erfolg molekularer Diagnostik generell und beim Handekzem im Speziellen.Es erfolgen die Diskussion von Grundlagenarbeiten und eine bewertende Zusammenfassung neuer Entwicklungen zur molekularen Diagnostik, insbesondere des Handekzems.Ein erster molekularer Klassifikator zur Differenzialdiagnose von (Hand‑)Ekzem und Psoriasis ist als CE-IVD (In-vitro-Diagnostikum) eingeführt, und viele weitere Biomarker zu Diagnose, Therapieansprechen oder Prognose des klinischen Verlaufs sind derzeit Gegenstand der Forschung.Die Diagnostik des Handekzems wird künftig ergänzt werden durch molekulare Diagnostik als Entscheidungshilfe; wir stehen am Anfang eines molekularen Zeitalters in der Dermatologie.
