A monocular high myope, post-trabeculectomy 8 years ago developed macular hole-related retinal detachment. She underwent pars plana vitrectomy with silicon oil (SO) tamponade. Post-operatively due to extensive subconjunctival SO migration via trabeculectomy ostium, there was an intractable increase in intraocular pressure (IOP).[1] This diffuse-illumination slit-lamp image shows subconjunctival SO in the area of the bleb and an early superonasal staphyloma at the sclerotomy site. The subconjunctival SO gives a bubble-wrap appearance [Fig 1]. Following SO removal, the retina was attached and IOP was stabilized with antiglaucoma medications.
