•Mycobacterium avium complex (MAC) lung changes were scored in serial CT scans.•The nodule (N), infiltration (I), cavity (C), ectasis (E) scoring system was used.•Nodules and infiltrations were reversible; however, ectasis was irreversible.•Early treatment may prevent progression of irreversible factors in MAC lungs.
