•Discrepancies between patient reported outcomes and clinician reported outcomes concerning urinary incontinence and erectile function after prostatectomy is demonstrated.•The discrepancy between patient reported outcomes and clinician reported outcomes with regard to urinary incontinence mainly concerned patients with mild and moderate urinary incontinence.•Clinicians significantly overestimated the recovery of erectile function (normal erection) in patients with IIEF-EF score 6 to 16.•Although clinicians did underestimate patient reported outcomes on functional outcome, management of symptoms was not affected.•Discrepancies between PROMs and CROs on functional outcomes did not affect management of symptoms.
