●Very few magnetic resonance spectroscopy (MRS) studies in transgender people exist.●Trans men, cis men and cis women completed MRS at 2 time points, 6+ months apart.●Trans men underwent gender-affirming hormone treatment (GAHT) during the 6+ months.●Testosterone treatment altered brain metabolites and ratios in parietal cortex.●Exploratory age of onset analyses indicated potential effects in amygdala.
