A Positive Doll's Eye Maneuver

A 22-year-old female was referred to the oculoplastics service for evaluation of a nonresolving chalazion. Panel A shows the normal right upper lid position; a focal area of fullness on the central upper eyelid can be observed (arrow). In panel B, eversion of the right upper eyelid shows a yellow capsule surrounding a darker object embedded in the palpebral conjunctiva and posterior lamella. Axial computed tomography (CT) scan demonstrated scatter artifact in the right medial preseptal tissues (panel C). Panel D shows the macroscopic appearance of the foreign body (0.8 cm in diameter) after excision with a flat posterior surface and a dark, loose pellet inside, consistent with an intact plastic doll’s eye. The patient could not recall any trauma involving a doll, and as she was adopted, her family was unable to provide a complete early history.
