•Surgery scheduling in a hierarchical diagnosis and treatment system is investigated.•A fuzzy optimization model based on a Stackelberg game is developed.•A hybrid algorithm for elective surgery scheduling fuzzy model is proposed.•The proposed algorithm is shown to be efficient against the state-of-the-art algorithms.
