1624 Phenotypic and endotypic determinants of atopic dermatitis severity from the ADRN registry

Atopic dermatitis (AD) has a highly variable clinical phenotype. For this US registry, AD participants (0.7-80 yrs) were enrolled at 9 academic centers. AD participants (N=2,862) were categorized as mild (11.6%), moderate (58.0%) or severe (30.4%). This cross-sectional study identified historical and clinical features as well as biomarkers associated with AD severity. Severity was strongly (P≤0.007) associated with a personal/family history of allergic disorders, exposure to passive smoke, dog ownership, ocular herpes, and skin infections. S. aureus skin colonization, serum IgE, aeroallergen and food (≤6 yrs) Phadiatops, and eosinophilia were also strongly linked to severity (P≤0.002). The ADRN registry significantly advances our understanding of the importance of AD disease severity in the development of several allergic and infectious comorbidities and identifies potentially modifiable risk factors.
