Primordial odontogenic tumor in a pediatric patient: rare case report

Primordial Odontogenic Tumor (POT) is a benign lesion of mixed odontogenic origin that mainly affects the posterior mandible of young patients. This report aims to describe a case of POT in a 12-year-old female patient presenting an asymptomatic left facial asymmetry. Intraoral examination showed swelling in the buccal vestibule of the left maxilla. Computed tomography revealed a hypodense, unilocular, and circumscribed lesion associated with an impacted tooth and extending from the left maxillary sinus to the orbital floor. The lesion was enucleated, and the involved tooth were extracted. Microscopically, we observed the proliferation of spindle cells in a fibromyxoid mesenchymal tissue surrounded by columnar odontogenic epithelium. The final diagnosis of POT was established, and the patient has been undergoing a 28-month follow-up with no signs of recurrence. Despite the indolent behavior, it is important to properly diagnose POT in order to avoid invasive surgical approaches.
