摘要
The incidence of AF is elevated in patients with cancer but the outcomes of catheter ablation in this patient population is not well-described. To assess the outcomes of AF catheter ablation in patients with history of cancer and further delineate the outcomes between solid tumors versus hematologic malignancies. From January 2019 to September 2022, all patients with history of cancer who underwent AF catheter ablation at a single quaternary center were retrospectively analyzed. Demographics, procedural details, and post-operative clinical course for one year were reviewed. Over three years, 309 of 2257 (13.6%) patients who underwent AF catheter ablation had history of cancer. 114 of 309 (36.9%) were female. 138 (44.7%) patients had paroxysmal AF, 114 (36.9%) had persistent AF, and 57 (18.4%) had multiple atrial arrhythmias with overall average CHADS2VA2Sc score of 2.8±1.6. 245 (79.3%) patients had solid tumors and 64 (20.7%) had hematologic malignancies. The demographics of patients with solid tumors versus hematologic malignancies are shown in Table 1. At one year follow-up, patients with hematologic malignancies had higher recurrence of AF compared to patients with solid tumors (Figure A, p=0.001). Acute procedural complications were documented in seven (2.3%) patients. The recurrence of AF following catheter ablation was noted to be higher in patients with hematologic malignancies compared to those with solid tumors. AF catheter ablation is safe in patients with history of cancer.Tabled 1Patient DemographicsVariablesSolid Tumors (N=245)Hematologic Malignancies (N=64)P-ValueAge69.5+/-9.263.9+/-11.5<0.01Female99 (40.4%)15 (23.4%)0.017Hypertension167 (68.2%)38 (59.4%)0.271Congestive Heart Failure74 (30.2%)21 (32.8%)0.29LVEF (%)55.2+/-10.751.3+/-14.30.02LA Diameter42.6+/-7.641.9+/-7.70.536Diabetes Mellitus37 (15.1%)10 (15.6%)0.453Obstructive Sleep Apnea75 (30.6%)14 (21.9%)0.166COPD33 (13.5%)7 (10.9%)0.446Paroxysmal AF114 (46.5%)24 (37.5%)0.282AF Recurrence47 (19.2%)24 (37.5%)0.001AF: atrial fibrillation; COPD: chronic obstructive pulmonary disease; LA: left atrium; LVEF: left ventricle ejection fraction Open table in a new tab
