This study aims to present a patient with an advanced lung adenocarcinoma (T3N2M0, IIIB stage), which was successfully downstaged after conducting four courses of neoadjuvant chemo-immunotherapy. The subsequent right intrapericardial pneumonectomy with partial resection and plasty of a left atrium showed a complete pathological response.
