Large-bore aspiration thrombectomy for acute PE is associated with favorable technical outcomes, but the concern for acute cardiac decompensation is non-negligible in subjects presenting with high-risk features and PASP ≥ 70 mmHg. ECMO can help salvage some of these patients and should be considered in the treatment algorithms of patients deemed at high-risk.
