•Grade group reclassification occurs in some men on active surveillance for prostate cancer.•Greater number of cancer locations (TCLo) is associated with grade group reclassification.•A large proportion of men with high TCLo density were found to have higher grade disease.•Men with low TCLo density had better progression-free survival.
