•Aberrant keratin expression in acute leukemia is extremely rare and may pose a considerable challenge for the pathologist.•Patients with B-ALL receiving blinatumomab have been reported to lose B-cell differentiation on disease relapse.•Extramedullary relapse of B-ALL is a rare presentation with poor prognosis.
