•IMV tumor thrombosis is a rare condition in CRC.•CRC with IMV tumor thrombosis is prone to hematogenous metastasis.•CRC with IMV tumor thrombosis may be a risk factor for R1 resection.•Systemic chemotherapy should be considered for CRC with IMV tumor thrombosis.
