•Cerebral small vessel diseases affect the intracerebral hemorrhage (ICH) prognosis.•A relation between the prognosis of ICH prognosis and old lacunes remains unclear.•We assessed the impact of old lacune and their locations on the ICH prognosis.•Old thalamic lacunes contralateral to ICH were associated with poor outcomes.•Old thalamic lacunes contralateral to ICH may delay the motor recovery of ICH.
