•Major depressive disorder with gastrointestinal symptoms suffered more cognitive and neuro dysfunction.•Using proton magnetic resonance spectroscopy to assess brain biochemical metabolism.•MDD patients with GI symptoms exhibited lower Cho/Cr ratio in right thalamus•MDD patients with GI symptoms exhibited lower NAA/Cr ratio in left putamen.•Brain biochemical metabolism changes in specific regions were correlated with GI symptoms.
