•Type A acute aortic dissection (AAD) with coronary malperfusion is a fatal disease.•Prognostic impact of PCI in this setting is unclear.•Primary PCI may improve survival in type A AAD patients with coronary malperfusion.•The PCI effect was primarily seen in patients with left coronary artery involvement.
