Transcatheter aortic valve replacement (TAVR) is now the mainstay of treatment for patients with severe aortic stenosis (AS) across all surgical groups. There remain a large proportion of patients who are ineligible for transfemoral approach due to peripheral vascular disease which precludes delivery of large bore delivery systems. The use of intravascular shockwave lithotripsy (IVL) as an adjunct to facilitate transfemoral access in otherwise unsuitable anatomy has shown promising results. We report our early multicentre experience from Australia.
