A rare case of disseminated genitourinary tract tuberculosis complicated by emphysematous prostatitis and seminal vesicle abscess

We discuss a case of extensive tuberculosis (TB) infection of the genitourinary tract presenting with epididymo-orchitis, renal parenchymal infection and emphysematous prostatitis. Scrotal ultrasound findings and a high-risk country of origin raised early suspicion of urogenital TB (UGTB), a diagnosis supported by further crosssectional radiological findings and laboratory tests. The patient was successfully treated by the Infectious Disease team with Urological input. TB rarely presents with genitourinary disease, tuberculous prostatic abscess is very rare and tuberculous emphysematous prostatitis is a previously unreported presentation; we discuss the clinical course, radiological and microbiological features of this case in addition to the wider role of radiology in the diagnosis of UGTB.
