Should we always retrieve? Endovascular treatment outcomes in emergent large-vessel occlusion due to underlying intracranial atherosclerotic stenosis

•For treating large vessel occlusion due to underlying ICAS.•To compare outcomes between SRT and first stenting without retrieval (FRESH)•Symptomatic ICH was significantly lower in the FRESH group.•Favorable functional outcomes were significantly more frequent in the FRESH group.•First stenting without retrieval (FRESH) could be a feasible solution.
