•/This is a primary case of panuveitis induced by donor-derived CD8+ T lymphocytes in a patient with aggressive ATL after allo-HSCT.•/Pathological diagnosis of vitreous infiltration by vitrectomy is helpful in patients with ATL to determine the phenotypes of cells infiltrating the eye of ATL patients.•/Donor-derived CD8+ T lymphocytes-induced panuveitis is recurrent but susceptible to regional corticosteroid treatment.
