•Neurosurgical access is geospatially varied within the United States.•The Great Plains, Rocky Mountains, and Southwest have the lowest neurosurgeon access.•Mid-Atlantic, Central Texas, and Southern Montana have greatest neurosurgeon access.•Rural-adjacent neurosurgeons service patients from surrounding, low-access counties.•Urban areas with many neurosurgeons often have proportionally less access per person.
