BK virus nephropathy (BKVN) is a potential cause of renal allograft dysfunction and loss in kidney transplantation recipients (KTR). BK viruria (BKVu) occurs in 30-40% of the KTR, while BK viremia (BKVb) and BKVN occur in approximately 13% and 8%, respectively. High-level BKVu can progress to BKVb, and BKVN. Early screening for BKVu and reduction in immunosuppression in BKVu positive patients may prevent the progression to BKVN and may improve long-term patient and allograft outcomes.
