•Primary lung carcinomas are very rare tumours during childhood.•Patients with mucoepidermoid carcinoma have a favourable outcome after resection.•Patients with other histotypes need new multimodal therapeutic strategies.•ALK inhibition in ALK-altered adenocarcinomas may improve the outcome.•Molecular tumour profiling should be systematically performed at an early stage.
